JAMZ Fall 2017 – Spring 2018 Recreational Contract

The undersigned adult student or parent/guardian acknowledges and understands that, upon JAMZ Dance Studio's acceptance of this registration/contract, Dancers are enrolled, and the undersigned Student or Parent/Guardian is financially responsible for the tuition for the entire 10 month session, commencing on September 1st, 2017 and continuing through the end of June 2018 ("Dance Session").

Registration Fee

An annual, non-refundable registration fee (per family) is required for all students registering for classes.  The fee is $35.00 for returning families, and $40.00 for new families.

Tuition and Payment

After initial payment of registration & September tuition, tuition will be processed on the first of every month, starting October 1, 2017.  The first month of tuition, and registration fee payment, are due upon signing and accepting of this contract.   A 5% late fee will be assessed to any tuition or fees not paid on dates due.

Monthly dance tuition is based on a guaranteed minimum of thirty (30) hours of instruction provided over a ten month period from September through the end of June (see studio calendar for details). Total school-year tuition is divided into ten equal installments regardless of how many classes take place in a calendar month, payable on the first of each month in advance via automated credit card, debit card, or automated check.

Families without or choosing to not pay with a credit card must provide a series of post-dated checks for tuition and fees dated for the first day of each month due (October 1 to June 1).  Checks should be made payable to JAMZ Dance Center, LLC.  No class schedule will be confirmed without receipt of a credit card number, or all post-dated checks.

Returned checks will be charged a $30.00 returned check fee per number of times the bank attempts to deposit your check (Often the bank will try to clear a check twice, resulting in a total returned check fee of $50.00). A returned check may result in JAMZ requiring future tuition to be paid via credit card, cash, or money order on or before the first of each month.  Failure to do so will result in removal from class(es).

Please note: JAMZ Dance Studio does not mail out monthly invoices for tuition payments.  At any time you may log in to your account online via our website to review your balance and see posted charges.  Please ask the front desk staff to email a temporary password if you have forgotten yours. We can also email you a copy of your account statement upon your request.

_____ (Initial)  All customers MUST have a zero balance on their account by the date of showcase.  Failure to do so will result in student(s) being withheld from performing in the show.  ONLY CREDIT CARDS AND CASH WILL BE ACCEPTED AS PAYMENT AFTER JUNE 1, 2018.

JAMZ has retained a collection agency to assist in obtaining past due balances, and will pursue this avenue in the event of non-payment.

When registering students for a class between the 1st and 15th of the month, the customer is responsible for the full tuition payment of that month.  Customers who register after the 15th will only be responsible for half of that monthly tuition payment.

Class rates are per student and are not discounted for multi-student families (example: Family with two children enrolls each in two classes.  Each student is charged $65 per first hour, and $60 per second hour).

Discounts:  Receive a 5% discount if paying for tuition for the full term in advance (tuition only; does not apply to other fees such as costume, showcase, etc).  Military/First Responders and their immediate families receive a discount of 10% off of monthly tuition only (cannot be applied to costume & showcase fees, etc).

* Drop-In classes: $20.00 a class, depending on availability.  Please call first to see if we have space in the class you are interested in.

* _____  (Initial) if taking Acrobatics Class Acrobatics: These classes are technique only, and will NOT be featured in the end of year showcase.  Acrobatics classes should be taken to complement other dance classes.  The Showcase Fee section does not apply to students/families solely taking acrobatics at JAMZ.

* _____  (Initial) if taking Ballet Class (ages 9+).  BALLET TUITION.

Ballet classes categorized as for ages 9 and up are 1 ½ hours in length. The cost per class is the hourly rate above, plus an additional $25.  For example, if you are only taking ballet, the cost per month would be $90.  If ballet is your “second” class, your tuition would be $65 for the first class, and $85 ($60 + $25) for the ballet class, for a total of $150.00/month.  For students in Ballet/Tap (ages 3 to 5), and Ballet (ages 6 to 8), the normal hourly rate would apply ($65, $60, etc).

There is a two-month minimum for all classes.  One-month notice from the first of the month is required to discontinue any classes.  Withdrawal must be done in person and will not be accepted over the phone.  Withdrawal must be done at the front desk and not with the teacher.  Withdrawal must occur within the first 7 days of the month (example:  withdrawing November 1st through 7th will result in only being charged for November.  Withdrawing November 8th or later will result in a full tuition charge for December).

_____ (Initial)  The last day to withdraw from classes is February 1, 2015.  No withdrawals will be accepted after February 1st, and you will be charged for the remainder of the school year tuition through June. To withdraw from classes, a parent must:

1.  Inform the school administration in person, as well as

2.  Complete and sign a withdrawal form provided by the front desk.

All automatic bank debiting or credit card charges will stop after the one-month notice period.

The school reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes.  JAMZ also reserves the right to change teachers during the school year for any reason.

JAMZ “caps” class size for most classes anywhere from 8 to 20 students depending on the style of dance.  However, we also have requirements with regards to the minimum number of students necessary to be enrolled to teach a class for the entire term.  Minimum requirements are not published.  We reserve the right to combine and/or cancel classes.  (example: should only two students sign up for a class, that class will either be asked to combine with another, or it will be cancelled altogether).  JAMZ will release the parent or adult student from this contract should we cancel a class due to the minimum size requirement not being reached, and provide a prorated refund for unused lessons.

A Showcase contract will be distributed in the first part of November containing full details of showcase participation.  This contract must be signed and returned by December 1, 2017.

Costume fees for the year-end dance showcase ($75 for classes designated as up to age 8, $90 for classes categorized as ages 9 and up) will be charged to your account on January 15, 2018. No costume fees will be refunded after this date.

_____ (Initial)  Due to budgetary constraints, families may be asked to purchase additional accessories including but not limited to:  a specific type of inexpensive shoe, earrings, etc.

A showcase fee of $60 per family will be charged to your account on May 15, 2018. Each participating family will receive two tickets to the showcase and one showcase DVD. Additional tickets (as available) are $17.50 each and additional videos are $35 each.  Tickets to the showcase will be sold on a first come, first served basis in late May/early June at the front desk.

_____ (Initial)  There may be two showcases at the end of our 2017-18 school year.  Families affected by having a student in both shows will be offered a “Golden Ticket” with their showcase package allowing passage into both shows for no additional charge.  Students enrolled in multiple dance classes may end up being in both shows.  While we will do our best to keep students to just one show, this very well may not be possible, particular with families having two or more students attending JAMZ.

Additional tickets (as available) are $17.50 per show, $27.50 per Golden Ticket; and additional Showcase DVDs are $35.  Tickets sold to the showcase will be sold on a first come first serve basis starting in May 2018 (date TBD).  Golden Tickets will be available in very limited supply.

A showcase information packet will be distributed in the first part of November containing full details of showcase participation.  This packet must be signed and returned by December 1, 2017.

All dancers participating in the showcase are expected to attend all rehearsals.  Rehearsals may be on a different day or time than the regularly scheduled class.

Expect the week prior to showcase to require mandatory nightly tech & dress rehearsals.

In case of inclement weather, JAMZ Dance Studio DOES NOT FOLLOW the Anne Arundel County school system.  Please call the studio and/or check the website for the most up to date information (www.JAMZdancecenter.com).

Our calendar has built-in extra rehearsal dates in the event of closures.  Often, the week before our showcase, classes will be two or three hours in length (not including dress rehearsals).  This ensures you are receiving above and beyond the hours you are contracting and paying for.

In the unlikely event JAMZ is in danger of not meeting the guaranteed minimum of thirty (30) hours of instruction, extra rehearsals will be added to the calendar.  Otherwise, JAMZ will not offer makeup classes.

Required dance attire and dance shoes must be worn to all classes. Failure to wear required dance attire to class could result in students being asked to sit out the class. Repeated failure to wear required dance attire could result in termination of lessons and forfeiture of tuition. Students are not permitted to wear jeans or shoes worn on the street or outdoors to any dance class. See our “Dress Code” for more information and specifics for recreational classes.

It is the responsibility of the parent, guardian, or adult student to be aware of all school activities such as: rehearsals, recitals/showcases, picture days, extra classes, and dates the school is open and closed. The school will provide monthly email newsletters with all information needed for upcoming months, and frequently updates its website and social media.  It is the parent’s or adult student’s responsibility to regularly check these emails and/or studio website to ensure they are informed.  Most of our communication is done through email (including receipts for payments).  No email should ever be overlooked and all should be read thoroughly.  JAMZ Dance Center is not responsible for updating people through paper communication or personal meetings.  It is also the responsibility of the parent, guardian, or adult student to inform the school of any address, phone, or email changes.

____  (Initial) I understand JAMZ communicates almost entirely through email, and it is my responsibility to notify the front desk if I am not receiving newsletters, receipts, and other emails.  I also understand it is not the responsibility of teachers to distribute information.

Attendance and Lateness

Late students may be asked to observe their class rather than risk injury due to lack of proper warm-up time. Repeated lateness may result in termination of lessons or class transfer. A minimum attendance level may be required for showcase participation.  If a showcase participant misses more than four classes without written notices, the school reserves the right to deny the student showcase participation.

Care of Students

The school cannot provide before or after class care for students. Parents with students under the age of 6 are to remain in the school during the classes.  Students are not to be left at the school for excessive time before or after lessons. Please provide a cell phone number in case of emergencies.

The undersigned, in her/his capacity, as well as on behalf of the Student, both of his /her Parents and Guardians, recognizing the risks of illness and injury inherent in any dance program (1) acknowledges and agrees that the Student is participating upon the express agreement that illness and/or injury may occur; and (2) agrees to herby release, indemnify  and forever discharge JAMZ Dance Center, its employees, agents, directors, officers, stockholders, sponsors, and volunteers, and their respective heirs, personal representatives, administrators, predecessors, and assigns (“indemnities”) from any all action, cause or causes of action in law or in equity, suits debts, contracts, agreements, promises, liability claims, demands, damages, loss, cost or expenses of any nature whatsoever, known or unknown, which the undersigned,  in her/his capacity, as well as on behalf of the Student both of his/her Parents/Guardians has had, could have, or may have, own or hold against any of the Indemnities by reason of the Student’s participation in the Dance Session or any other JAMZ Dance Center program or event, except for illness or injury resulting from gross negligence or willful misconduct on the part of any of the indemnities.  I hereby agree to execute and deliver this Waiver and Release to induce JAMZ Dance Studio to permit the Student to participate in the Dance Session and other JAMZ Dance Studio programs and events.  I hereby grant JAMZ Dance Studio and its authorized designees permission to administer first aid and/ or call 911 in case of a medical emergency affecting the student.  I acknowledge and understand that JAMZ Dance Center will attempt to first notify parents and guardians in case of an emergency.

Photo Release

JAMZ Dance Studio, LLC is hereby granted permission to take photographs and video of the students to use in social media, brochures, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.  The release will allow us to videotape our Showcase as well as record in class footage to share video for students to rehearse choreography.

Failure to agree to either condition above will result in the inability to complete registration and enrollment.
